Questions & answers not appearing on SmartForm submissions

Unanswered questions do not appear within the submission of a SmartForm

Unless the question is mandatory, any unanswered questions will not appear within the submission of a SmartForm.

This was designed specifically to help minimise the clutter of the output

The easiest was to check what questions are asked is by working through the SmartForm as though you are a Patient, remember to complete this for both Male & Female options if present.

The most common occurrence of this is the NHS Number

Whilst this can be made mandatory, you may find that patients complain as the guidance for registering for a GP states.

You do not need proof of address or immigration status, ID or an NHS number.
